What you’ll need
For your first visit with us:
- A referral from your practitioner. (find a downloadable copy here.
- Patient information form for pre-consultation (approximately 20min. to fill up online)
- Copy of approval (if workers comp/CTP)
- Scans, x-rays, or recent investigations & reports
We will be in contact with you within 1 week of receiving the ‘Patient information form for pre-consultation‘ to organise an appointment for you.
If you are requesting a follow-up appointment and it has been less than a year since your last visit:
- Make sure you have a valid referral / approval (validity of 1 year from GP’s and 3-6 months for specialists)
- You can send your updated referral here : contact@sydneyspinepain.com.au after filling up the request for appointment form.
If you are requesting a follow-up appointment and it has been more than a year since your last visit
- Please follow the process as for a new patient inquiry – we will need a valid referral and you to re-fill the Patient information form for pre-consultation here: https://sydneyspinepain.snapforms.com.au/form/ssp-patient-authority-pik
If you require medical attention please contact your general practitioner or in the case of an emergency your nearest hospital.
